Rather than delivering a power fantasy, it makes survival feel desperate, confusing, and deliberately uncomfortable.

Most of the campaign plays as a first-person stealth adventure built around observation, hiding, and careful movement through hostile areas. You search for routes, solve environmental puzzles, interact with stranded souls, and avoid demons that can end a run quickly when you make too much noise or wander into view. Its defining mechanic is possession, allowing the protagonist to seize control of certain humans and weaker demons; losing a body is not always the end if another suitable host is nearby. Checkpoints and limited resources make progress tense, while the game’s obscure clues and unforgiving encounters demand patience more than lightning-fast combat reflexes.

Agony stands apart through the sheer commitment of its infernal visual design, presenting Hell as a surreal biomechanical nightmare instead of a familiar fantasy dungeon. The story campaign offers a guided descent through that setting, while an additional challenge mode generates randomized areas for score-chasing survival runs, giving players a reason to return after the credits. Expect rough edges, including occasionally unclear objectives and uneven stealth behavior, but the atmosphere often carries the experience when its systems falter. Fans of disturbing first-person horror such as Outlast, Amnesia, or Scorn who value oppressive imagery and strange world-building over polished action will find the most to appreciate here, while newcomers to harsh stealth horror may find it punishing.
