Set inside MOTHER, a colossal self-replicating structure that has all but consumed the universe, ALTEREDBLOOD+ casts you as a purpose-built Reaper hunting parasitic invaders in her depths. Its mood is aggressively alien and apocalyptic, pairing grotesque organic architecture with breakneck arena combat. The result feels like a collision between classic PC shooters and a splatter-driven territory system, where every kill physically changes the room around you.

Fights revolve around deliberately spraying enemies’ blood across walls, floors, and nearby surfaces, then using that spill to sustain your momentum. Sliding, dashing, and stomping through blood restores health and ammunition, while standing in it alters both your body and your current weapon with more dangerous properties. Positioning matters as much as aim: a poorly placed kill can leave precious resources out of reach, while a well-directed blast creates a route to escape, heal, or press an attack. Movement is equally central, with bunny-hopping, air-strafing, surfing, trimping, rocket jumps, and selectable classic or modern air-control settings giving skilled players plenty of speed to exploit.

Story missions steadily introduce weapons, foes, and the strange rules governing MOTHER, shifting between tight, high-pressure clashes and broader combat spaces designed for aerial movement. Gravekeeper mode then turns those tools toward survival runs against escalating waves, with weapons unlocked through the campaign carrying over into the challenge. Leaderboards add a competitive reason to refine routes, scores, and survival times, even though the heart of the experience is mastering its single-player combat sandbox.
