A flooded southern Chinese town becomes the setting for a personal nightmare in LakeLady, a story-driven horror adventure built around grief, buried guilt, and occult secrets. Salvio returns home as a doctor after his daughter disappears in a devastating flood, then uses his position to investigate Hongqiang Hospital and its increasingly suspicious staff. Missing children, a sealed Ward No. 9, and manifestations tied to traditional spiritual beliefs turn a grounded tragedy into something far more grotesque. The mood is humid, melancholy, and unnerving rather than relentlessly action-heavy, with 1990s details giving the hospital and town a convincingly lived-in sense of place.

Most of the experience revolves around talking to suspects, examining environments, gathering useful objects, and making decisions that shape which leads remain available. More than ten major characters have conflicting motives, and conversations can involve persuasion, distrust, or choices that alter relationships and expose different sides of the mystery. Gameplay shifts between side-scrolling exploration and escape sequences, medical procedures presented through a surgery simulator, and exorcism encounters based on Five Elements concepts and talismans. Puzzles aim to support the narrative instead of stopping progress with arbitrary riddles, while danger comes from tense situations where the right information or item can determine whether Salvio gets closer to the truth.

Its strongest feature is the way branching routes make one complete run feel intentionally incomplete; missed scenes and withheld clues can radically change how players interpret people and events. Traditional Chinese occult imagery gives the horror a distinctive identity, while the old thermoses, public phones, worn furniture, and rain-soaked atmosphere evoke a specific millennial-era nostalgia rarely seen in horror games.
