Set during the final stages of World War II, the narrative follows Giulia after the body of her twin sister Martha is recovered from a lake, forcing her to confront a family tragedy nobody seems willing to explain honestly. The horror is less about constant chases than dread, disturbing imagery, and the unreliable nature of memory. Italian voice acting gives its 1944 setting an unusually grounded texture, even as the story grows increasingly surreal and upsetting.

Much of the experience revolves around investigating locations, examining clues, reading letters, and piecing together what Giulia sees against what may actually have happened. A detailed in-game camera plays a meaningful role: players take photographs, develop film in a darkroom, and use pictures to document clues and advance the investigation. Tuscany is open enough to travel through on foot, bicycle, or rowboat, with farms, forests, cemeteries, and villages providing a quiet but ominous backdrop to the conflict closing in around the family. Progress is primarily narrative-driven, with environmental puzzles, scripted sequences, and puppet-theatre scenes used to revisit buried memories rather than traditional combat or survival systems.

Martha Is Dead stands apart through its willingness to make its protagonist and its audience uncomfortable; it handles trauma, fascism, bereavement, and self-deception with little interest in easy answers. Its idyllic countryside and carefully recreated period details contrast sharply with some exceptionally graphic scenes, so it suits players who can tolerate intense material in service of a bleak story. The pacing resembles narrative mysteries such as What Remains of Edith Finch or The Vanishing of Ethan Carter more than a combat-focused Resident Evil, though its psychological unease is far harsher. Fans of The Town of Light, historical horror, and slow-burning investigations will find a memorable, flawed, and often genuinely unsettling descent into fractured truth.
