Milanoir follows Piero, a newly released convict who discovers that the betrayal which put him behind bars is far from over, leaving him hunted by criminals, corrupt forces, and old enemies. Its mood draws heavily from the rough Italian poliziotteschi films of the 1970s: cheap suits, smoky rooms, sudden violence, and nobody trustworthy for long. A pulsing funk soundtrack gives the bloodshed a stylish, grimy energy rather than treating the city as a realistic crime simulator.

Piero fights his way through compact story-driven stages by shooting armed thugs, grabbing foes at close range, choking guards, and using stealth when a direct assault looks suicidal. Encounters reward quick reactions and aggression, but limited room to maneuver means careless players can get trapped in crossfire or overwhelmed by enemies entering from multiple sides. The campaign breaks up its on-foot brawls with frantic vehicle sequences, turning narrow streets into dangerous escape routes where survival matters as much as landing hits. Local two-player co-op lets a second player join as the Guy in Blue, making the action more chaotic and giving friends a way to tackle the campaign together.
