Painkiller Overdose puts players in control of Belial, a half-angel, half-demon outcast fighting through the afterlife after being rejected by both sides of its eternal war. The tone is gleefully excessive: ruined cathedrals, industrial nightmares, lava-filled arenas, and grotesque creatures all exist to be shredded in a storm of rockets, stakes, and supernatural fire. It is an old-school FPS built around constant forward momentum rather than cover systems, scripted stealth, or long conversations.

Combat revolves around clearing hostile arenas, hunting for keys and exits, and keeping an enemy-killing rhythm alive while foes swarm from every direction. Belial carries a roster of six distinctive demonic weapons, each with alternate attacks, so the familiar stake-launching, rocket-blasting chaos gains tools such as corrosive projectiles and close-range crowd control. Enemies range from fast skeletal nuisances to hulking monsters that demand strafing, jump timing, and smart weapon switching, while oversized bosses turn the finale of many levels into a test of movement and ammunition management. Single-player progression spans a substantial collection of supernatural locations, and multiplayer adds a more traditional deathmatch-focused outlet for players who want to bring the arsenal against human opponents.

What separates Painkiller Overdose from many retro shooters is its surreal horror atmosphere and physics-heavy carnage: bodies fly, enemies pile into arenas, and every weapon feels designed for spectacle as much as efficiency. The campaign does not chase realism or narrative depth, but Belial gives the mayhem a stronger central character than a simple anonymous marine setup. Visuals and interface design show their age, and the difficulty can become punishing when elite enemies flood a confined space, yet the directness remains its strength.
