Isaac begins with little more than tears as ammunition, but survival quickly turns him into a bizarre walking arsenal. Its hand-drawn, crude-looking style is intentionally uncomfortable, matching a story built around fear, guilt, family trauma, and nightmare logic.

Each attempt sends players through a newly arranged sequence of rooms where they dodge enemy patterns, clear combat encounters, hunt for keys and bombs, and choose which risks are worth taking. Treasures radically alter Isaac’s attacks and body, producing combinations such as homing tears, lasers, orbiting shields, explosive shots, familiars, and effects that can turn a weak start into an absurdly powerful build. Shops, secret rooms, challenge chambers, devil deals, and powerful boss fights make resource management as important as raw reflexes; sacrificing health for forbidden upgrades can save a run or ruin it. Death means starting over, but completed runs gradually unlock additional items, characters, bosses, routes, and endings, ensuring that persistence constantly expands the pool of possibilities.

What separates it from many roguelikes is the sheer volume of item interactions and the willingness to let chance create both brilliant synergies and spectacular disasters. Runs are short enough for a quick session, yet dense with secrets, alternate paths, and hidden unlock conditions that invite hundreds of hours of experimentation. The original release feels rougher and more limited than later expanded editions, but its harsh Flash-era presentation gives it a distinct raw character.
