A return to Sehwa’s nightmare realm turns an ordinary school day into another desperate fight for survival. The Coma 2B: Catacomb follows Youngho Choi after his apparent escape from the shadow version of reality known as the Coma, only to find that its horrors still have a hold on him. His search for pages from Yaesol’s writings leads beneath Dokaebi Market, toward a relic weapon and answers connecting the first game to The Coma 2: Vicious Sisters. The mood is tense, oppressive, and distinctly Korean in its urban folklore, school corridors, and eerie marketplaces.

Youngho navigates a 2D side-scrolling district packed with locked routes, environmental puzzles, clues, and scarce survival supplies. Progress depends on searching rooms, gathering useful materials, trading for emergency items, and finding tools that open shortcuts or previously unreachable sections. A relentless killer patrols the area and reacts to noise and visibility, so sprinting recklessly or keeping a flashlight active can turn exploration into a frantic chase. Hiding spots offer temporary safety, while afflictions and limited resources force players to decide whether a risky detour is worth the reward.

Hand-drawn environments and comic-style story scenes give Catacomb a memorable identity, balancing colorful art with grotesque imagery and constant dread. Rather than being a standalone scare ride, it works as an important connective chapter: returning players get more context for Youngho, Yaesol, and the Vicious Sister, while newcomers still receive a focused survival-horror mystery. The expanded market trading and more aggressive pursuer make moment-to-moment survival feel less passive than in the original The Coma.
